Voicemail trouble

I am having multiple problems with my voicemail.

1) The notification will appear at random, even when I have no voicemails. I found that if you call from another phone, leave a vm, listen to the vm, and delete the vm, that will make the icon go away. This happens about once per month. How do I stop this from continually happening?

2) Recently my voicemail stopped working completely. When someone calls it continually rings without ever going to voicemail.

Does anyone know how to correct both of these?

What make/model phone/device do you have? Also, what voicemail service in coming into play here?

Hi Joe. It is a Samsung Note 8. I'm using the Basic Visual Voicemail.

Regarding the first issue, have you ensured that your voicemail app is up to date? Also, have you tried resetting the app by clearing its data? http://vz.to/2fdb2Aa

Since this issue started have you dialed *73 from your phone to ensure that Call Forwarding is not active on your line? This feature can accidentally be enabled and is usually why calls to your line would not go to voicemail at all.
