Splitting accounts

I just split up with my crazy girlfriend.

We have a contract together where she is the account owner, and I'm a member. She is keeping me on the plan as a method to keep tabs on me and stay in control with any way she can.

Needless to say I need to isolate my phone and account from hers before she charges me with extra things, claims my phone is stolen, or find myself without cell phone service one day at her command.

She is nuts.

How can I do this, essentially without bringing her back into the picture in any way?

If you want to keep your number and phone device, your options are Transfer your service FAQs | Verizon Wireless. You will have get the account owner's permission to separate your line and phone including the device payment agreement from the existing account.

If you are willing to give up your phone number and device, you can start your own account with a new number. I would recommend you get a new phone as well. You should then transfer your information from the old phone to the new phone and factory reset the old phone before shipping it back to your ex. Since they are the account owner  they are technically financially responsible for paying the service and the device payment agreement. If you were paying your portion for the device payment agreement, I would also suggest that you include the remaining amount owed when you ship the phone back to her.
