Setting up Fios in Windows Vista

When I use the connection wizard it asks for an ISP provided Username and Password. I don't know what they are asking for, any one know?

It is asking for your Verizon username and password that you setup when the tech installed your fios, It is the same as your e-mail without the @verizon.net on the end.
