Never Received Fios Welcome bonuses

I signed up for Fios back in January of this year. I signed up to receive the Visa Gift card rewards after my service was active long enough. Had to call in and got two email confirmations that it was being processed. I got the emails back in April that my reward was on its way. Never received them.
