Damaged GTE Utility Box

I work for the County of Maui Public Works Division. There is a damaged GTE Utility Box located on the sidewalk near 300 Maalaea Rd., Wailuku, HI, 96793. Two people have reported being injured by the uneven utility box cover. Can you please send some one to fix this issue before anyone else gets hurt. Thank you
