Verizon login form broken on HBO GO device activation page

I'm trying to activate a Roku device for HBO GO.

I do this:

Visit http://www.hbogo.com/activate/#/affiliates

I select "Roku" as my device.

That redirects me to the "Affiliates" page.

http://www.hbogo.com/activate/#/affiliates

I click "Verizon Fios" from the list.

The verizon login dialog appears.

I enter my information and click "Sign in".

Instead of proceeding to the next step, nothing happens.

I opened up the error console and see these errors, indicating an error on the verizon login form.

VOLPortalLogin:190 Uncaught ReferenceError: dom is not defined
VOLPortalLogin:1 Refused to display 'https://www.verizon.com/foryourhome/MyAccount/ngen/upr/signin.aspx?session=…2Fprofile%2Frest%2FprofileService%2Fafflogin%2FVERIZON%3FserviceCode%3DHBO' in a frame because it set 'X-Frame-Options' to 'SAMEORIGIN'.

I assume this is Verizon's script.

Please fix, or get HBO to fix, so I can activate my Roku device.
