Entire exchange down.....3rd time in 5 years. What's going on around here?

As the subject says, Pining Hills exchange cut off from the rest of the world. Can't call out of the exchange, have DSL synchronized but cannot pull an IP. A fiber cut no doubt but how can this happen so frequently to one area?
