2 (200) gift cards rebate for purchase of Internet Service

I noticed that my rebates were available up to 2 weeks ago and now that I am at the date for redeeming they have disappeared. I see this is an issue for lots of customers. I would like to get what I was promised, Verizon.

How can I get my 2 gift cards.
