Payoff phone from previous carrier

Where can I find information on how to submit the rebate to pay off device from previous carrier. I place an order and the sale person did take information from my previous carrier. He told me to go home and submit the rebate however I can't find any information on it. Please PN me for how to do it. Thank you.
