LG Cosmos 2 - intermittent calling problem

I just upgraded to an LG Cosmos 2 a few days ago. Occasionally (4 times over the past 5 days) I have called out and my phone didn't ring, nor could I hear the person when they answered. Apparently the call went through ok and the person on the other end could hear me.

 

Is this a malfunction of this particular phone, an undocumented "feature" of this phone, a problem with the verizon service, or some obscure option that needs to be set? I tried the external volume control ... to no avail. I'm not sure what to do when this happens other than hang up and call back. However, it is very annoying and I'm not sure I want to deal with this for the next 2 years.

 

If anyone has any suggestions, please let me know.
