My phone icon/app has mysteriously disappeared form my home screen main menu bar and has been replaced with the Kindle icon/app. How do I get my phone app back???

Press and hold the Kindle icon until you feel the confirmation vibration, then take your finger off of the icon. A list of apps will appear which you can replace that app with. The dialer should be in that list.
Edit: you must have done this inadvertantly to replace the dialer with Kindle.
