I accidently locked my phone. It's reading "SIM card is PUK-locked".
What do i do?

I'm having the same problem how do I know what my PUK code is?
The only way I know of to get unlocked is by taking your phone to a local Verizon store. They have the PUK (Pin Unlock Key) codes.
