If the PUK code has been tried to many times you will probally be forced to replace the SIM it the option doesnt allow you to reset the password... Have you tried https://nbillpay.verizonwireless.com/vzw/accountholder/services/myphoneSubmit.action and clicked the "Reset PUK Code"?
