My iPhone says something about PUK and will not make or receive phone calls, text, or data. Help!

The PUK can actually be found via My Verizon. Please follow the instructions set by tikibar1:

"Log into your on-line My Verizon account, then navigate to My Devices > Device Overview. On the right-hand side under About My Device, click on My PIN and unblocking key (PUK)."
