Why is my volume turning back on after setting my phone to vibrate or no sound??

Several times this morning I have turned the volume to off or vibrate and it's turned itself back on. I can't have my phone going off at work. How can I fix it without turning it off? I just recently (last Friday) received the newest update on my phone.
