Hard reset of LG revere flip phone

I need to hard reset my LG revere to send in for a rebate. I do not know the passcode and already tried the last 4 digets of my phone number.

Any suggestions?

Actually, I tried another pass code I have used in the past and it worked. So got it reset and sent off.

Let's make sure you can take advantage of your rebate. Have you tried dialing *22899? That is the first thing I would try to reset or remove the lock code.
