4G-LTE data was up and running in Boston yesterday and today (as I type this). So, it's not the network per se. I am using a USB modem on my notebook. Smartphones work differently in that they need services re voice and telephony, billing, etc. In 4g mode, the phone probably uses some kind VoIP technology to place calls (a la Skype). Since 3G and LTE operate on completely separate frequencies (1900 MHz for 3G and 700 MHz for LTE), devices need either two radios or some kind of smart switching (cheaper likely). I know my modem is pretty dumb when it comes to switching. I use the diagnostics mode of VZAM to access settings similar to the MiFi config page. Right now I'm in Global mode with mobile IP address preferred. Fixed IP only works in 1xRTT not in EV-DO-rev.A or LTE. Clearly, all these devices need to be much smarter about switching frequencies and modes. In 3G, these things are pretty smooth.
