The solution is easy:
If you want to go back to using VZ Access you need to specify IPV6 and include enough zeros 8 for an IPV6 address
at+cgdcont=1,"IPV6","vzwims","0:0:0:0:0:0:0:0",0,0
The Error is because you did not include enough 0's for the IPV6 parameter.

The AT command does take, verified with at+cgdcont?
but I still get the same wmc604 error
anybody actually get this to work?

When I first received my modem on the 5th of December it worked great with 4G but now I get the dreaded WMC604 error
I have tried all of the suggestions listed in this thread. I was able to get the terminal inputs to work as shown but I still get the WMC604 error when trying to connect to 4G.
When I plug in the modem it finds a 4G signal and it is at -66 dBm but then it seems to disconnect and search for a signal again. It does this over and over then it finally just settles for 3G.
I am able to connect at 3G without an problems but 4G is a no go right now
I guess I will go to Verizon Wireless tomorrow.

Is there any way to get the modem to connect 4G only and ignore 3G?

I tried the steps listed above but cannot get it to work. Has anyone else been able to get this to work in Windows 7?
I get an error 678 "The remote computer did not respond"
I enabled logging and created a diagnostic report if anyone wants to see it

Are you sure you are in a 4G area? the dial up won't work on 3G.(unless you change the authentication settings)
You can open vzaccess to check coverage and leave it open.
With vzaccess open the dial up connection on 4G should still connect you right up
I have also found that the user and password are not required.

I've been getting this on and off but I can usually connect w/o jumping through the AT command set hoops (jeez, I haven't done that since crosstalk/1988).
