3	Enter the appropriate information: Email address and confirmation Phone number assigned to the new 5G Home Equipment (find this under account details on the My Verizon app or vzw.com/myverizon) Confirm phone number First/Last Name
4	Submit the offer redemption within 60 days of activation.
