Billing error continues

On May 25th, I called to cancel my Verizon internet, but then changed my mind within an hour or so and called back to continue with the Verizon internet.  The CSR said no problem and he would reverse the cancelation.  A few days later, I received the box to return the equipment so I called Verizon again and asked about whether my request to continue service had been accepted. Again, the CSR said not to worry about it.  Two more calls to Verizon in June and again, I was told it would be taken care of.  Now, I have a notice of a $419 bill due next week for equipment not returned.  I have paid my bills in total and on time.   I will be making a 5th call to Verizon to speak with a Supervisor.  If its not resolved by July 12th, I will be cancelling everything  and going with someone else.  I am just about done with unfulfilled promises.
