Need equipment return label

I have not received my equipment return label for 5g home internet service. I have talked to multiple customer services reps but nothing has been received. I am frustrated and do not want to be charged $200 for something I am trying my hardest to return. Verizon, give me a return label

I'm in the same boat after contacting verizon for 5 months. I have been charged the $205 so now I'm trying to take it to the next level due to taking my money.
