Verizon not sending Return label

Hi , I called Verizon customer service more than 10 times since 4 months to send me return label , but still now I haven’t received. And surprisingly they charged me more than 200 dollars for not return fee for that router . I don’t know what I need do , how I can return this device and get my refund.
