Where is my return box for this router??

After having Verizon's 5G internet for 2.5 years, the service has been getting worse as time goes on so we canceled it over a week ago and went with another provider. After speaking with multiple service reps about the issues and the cancellation, I was told I would receive a return box in the mail. Low and behold, I have not received a box or a label so I can return the router to Verizon. After being with Verizon for over 15 years, if I get charged an equipment fee because Verizon failed to send me a way to return their equipment to them, I will be pulling the plug on all of my services and going to T-Mobile. Can a Verizon rep PLEASE assist me in getting the box and/or label I was told almost two weeks ago that I would receive??
