I'm on an LG G4 using Message+.
Why does the phone (dial) icon disappear in Message+ when the phone turns horizontal? Is there a way to fix this behavior? I often have the phone in a car holder horizontally and it's far easier and safer to hit the dial button when I get a text that needs an urgent response, but Message+ isn't allowing me to do that when the phone is in the holder horizontally.

My color scheme is different, but I still get the phone icon when in landscape. (And you can resize the pix by highlighting and dragging the corners, or cropping and resizing before posting.)
I'm seeing a few differences, and I'm not sure if it's the "theme" or color scheme I'm using or differences in the app between your phone and mine - I have a Droid Turbo, and the app is v. 6.1.3. I don't see how customizing the convo bubbles or background color would affect your menu items, which I don't see at all (Messages, Photos & Videos, etc beneath the contact info and above the message convo.)
Are you using the app or just logging in to messages via MyVerizon?
