$200 for Eligible trade-in devices: iPhone 4, 4s, 5, 5c, 5s. Trade-in must be in good working condition. Gift card takes up to 2 weeks from receipt of trade-in.
This $200 gift card offsets the $199 price of the iphone6 or discounts the $299 price of the iphone6+

Good working condition - if the screen has a crack but the phone works perfectly is this good working condition?

Sorry, no. Needs to be fully functioning screen, with no cracks. Also no water damage or corrosion, and no cracks, dents, or missing parts or pieces.

How do we go about doing the trade-in online? If you want to pre-order the Iphone 6 and trade in a 5 is there going to be an option for that? Do we then mail the iphone 5 or take it in to a Verizon store?
As well, anyone know if that $200 gift card can be used to pay your bill?

I do know the gift card CAN be used to pay on your bill. Log in to your verizon account and act like your going to make a payment online. Under other forms of payment is the choice for gift card!!

Just an FYI.
From what the csr via the telephone call centers tel me the GC are tiered. The older phones are lower in value. The website post is vague in this regard.

So you get no dollar amount gift card if it's not in working condition? For instance, my volume down button doesn't work on my iPhone 5. Am I better off just claiming my $62 from the Apple Store?

If it's examined they could reject it. Online auctions or selling it locally are possibility. Or go to your vzw store and have the sales associate look at the handset. They work on commission so they might put it through for you.
