If the replacement/new phone is another iPhone, and both devices use LTE (meaning iPhone 5 or newer), just take the SIM Card from your broken phone and put it into the new phone.

Otherwise, see this FAQ: Activate, Switch or Bring Your Own Device FAQs | Verizon Wireless. The top most option contains the steps you need to activate the phone.
