Where/how can I buy an iPhone 5s SIM card? And are there any procedures i need to go about to activate it or anything?

Is the phone unlocked and has bands to work with Verizon network?

you will need to sign up for Verizon service.

The 5s has the correct radios and band to work with Verizon.
