2FA - unable to access account to re-enable phone

I had to reset my iphone (e-sim reset), now I am unable to make calls. Unable to login to the Verizon website due to two factor auth. how do i fix this? 

Can i I update 2FA to use my email address?

HI Jesse,   I see the setup cellular options: 

Transfer from nearby Phone or use QR code.

I don't have a nearby phone (since this is the only phone)

so i chose the QR option.   Where do i get the qr code?
