COSTCO SHOP CARD 100 NEVER RECEIVED

We traded in an iPhone SE on August 2 for a new iPhone SE 3G.  The Verizon Kiosk offer was "Costco Shop Card 100".  We have not received it yet.

Costco Support said to call Verizon. I have been on hold with a Verizon rep for 40 minutes waiting for her to come back.  

Help???

jwpastor, we certainly understand how important it is to receive your expected promotions. The Costco Shop Card is sent as an ecard within 2-3 weeks after the 90-day return policy has expired. Was your purchase completed more than 90 days ago plus the 2-3 weeks following?

Thanks - I was able to call the Costco Shop Card Help desk at 888-668-9668 and they gave me an update.
