When I am on the phone and another person calls, my caller ID is suddenly disabled. The call waiting icon is also suddenly missing and has been replaced with a facetime icon. How do I restore these functions to my phone? I have tried rebooting and a system reset but it did not work.
Go to settings->general->reset and select Reset Network Settings. This will fix the call waiting/id issue and it will not delete any of your apps/media (it's not a factory reset).
That's an odd occurrence! I want to ensure your device is working as expected again.
Have you tried what "crelin" suggested? If so, let us know the outcome. I would've also suggested reprogramming the device by dialing *228 Option 1, then resetting the network settings http://vz.to/13oHZMX as well. Keep us posted!

I GOT A NEW IPHONE4 APRIL 24TH SINCE THEN I HAVE BROUGHT MY PHONE INTO THE VERIZON STORE TO HAVE THIS SAME PROBLEM FIXED. TWO TIMES. FIRST HAPPENED TO MY FRIENDS IPHONE, AND THEY FIXED IT THE FIRST TIME.
BOTH TIMES I WENT IN THEY TOLD ME THEY HAVE BEEN SEEING THIS ALOT WITH THE IPHONES LATELY, SO I WAS SURPRISED TO SEE YOUR COMMENT THAT THIS IS AN ODD OCCURRENCE.
BOTH FIXES LAST AT THE MOST 10 DAYS.... THEN WHEN I GET ANOTHER CALL, PHONE MAKES CALL WAITING NOISE, BUT WILL NOT SHOW WHO IS CALLING. I HAVE A PARENT AT HOME IN HOSPICE, AND IT IS IMPERATIVE
THAT I KNOW WHO IS CALLING. WHEN IT HAPPENED AGAIN TODAY, I WAS TOLD BY VERIZON THEY WOULD GET ME A REFURBISHED PHONE.
THIS IS NOT ACCEPTABLE TO ME. THIS PHONE IS 3 MONTHS OLD. I SHOULD NOT HAVE TO RECEIVE A REFURBISHED PHONE. I LOGGED ON TO VERIZON JUST NOW TO GET EMAIL CONTACT ADDRESS AS I WAS GOING TO WRITE TO VERIZON AS WELL AS APPLE TO LODGE MY COMPLAINT.
First stop writing in ALL CAPS
Now I agree about getting a refurbish piece of junk but that is what you receive under warranty.
I would bring the phone to an Apple store near you. Much easier than going through Verizon.

I am saddened to hear about the problems you are having with your phone. I would be upset if I had these problems too. The replacement option is the way to go. I know that it is not sitting well to get a Certified Like New Replacement. Well, that did not fix it, on the setting side and when you call 228 it kicks you off the network. So, how does one fix it.
Your right, one phone now. Hope to find an answer, probably Apple and Verizon's way to move us to the 5
I understand how important Caller ID and Call waiting are for our daily calls. I had same issue with another customer and we were able to resolve with a factory reset.
