Credit Card insurance claim

I am trying to file an insurance claim for a broken phone through my credit card. The credit card benefit service is asking for:

  • A copy of a diagnostic from an authorized service center stating what is wrong with the product and whether it is cost effective to repair it.

However, I turned in the phone to verizon already. Can I get verizon to issue a statement saying the phone was damaged beyond repair?
