Mobile + Home Rewards Issue

In January of this year, I upgraded my home internet service to Gigabit Connection 2.0 and switched my wireless service from ATT to Verizon.  I have since received all applicable Mobile + Home Rewards incentives for this internet/wireless service initiation except for a $100 credit that was being offered at the time.  I have been unable to find a suitable point of contact to investigate this and am reaching out via the community.  Can you help?
