Log into your on-line MyVerizon account.  At the top, hover over MyVerizon, then click View All under MyDevice.  Click on Share Name ID, make the necessary changes, then click Submit.

If your name is coming up as restricted as you make it sound, you may have caller id blocking on your phone which is just a feature on the line that needs to be removed. Share Name ID is also very handy to customize how your name appears to others with Caller ID.
