$200 rebate for switching never received

I switched to verizon early April and since that day I have been trying to get my rebate. I called and chatted with multiple CSRs and everyone has a different response. 

I have been to the store multiple times too but they tell me that they will open a ticket but they never do.

CSRs on phone said that they will open a ticket for me but they never do when I call back after few days.

Why so many lies? Like I switched for this reason. I paid more money to switch and now I am not even getting my rebate.
