Can't cancel in abroad without SIM and password

HI, 

I need someone's help with cancellation

 

I  left the States last spring, but I noticed I have been paying for my U.S. phone for almost one year.

 I have not used it at all, almost 1 year since I came back to my home country.

 

I already lost my SIM and forgot my account password.

So, I have no idea how to cancel my contract.

Could someone help me solve this problem?
