Cannot request a return box for cable card

We have moved and are no longer at the location on file with our old account.  Service is not available at our current location and had to be cancelled.  I am trying to request a return box and label for a cable card; however the website responsible for this has been down for days and the online system will not allow the request.  There are no return centers in the vicinity of our new location.  I would like to please request a return box sent to our new address.
