Fios + Mobile Discount not Being Applied to Fios Bill

My Fios bill fails to apply the $24.99 discount that should appear for users with both Verizon Fios/Mobile accounts:

This is the third month in a row that this issue has occurred.  I had to call support in July and August, and they were able to successfully apply the $25 adjustment:

As much as I love talking to support, I'd rather not have to call every single month to get this issue resolved. I'll eventually call them again, but figured I might as well post here too, in case anyone has dealt with this before.

To other forum users: has anyone experienced this before and had it resolved?

To Verizon: I have the INC number from my call in August. Please IM me.
