Fios Visa reward card never received

Hi, 

I was supposed to receive a $200 Visa card, but it has not come in the mail. The reward status is listed as shipped, with a mailed date of 10/13/2019. The "must-redeem by" date is 12/72019, so I'm getting a little worried that I will not receive the card in time to meet this deadline.

Any help in this matter is greatly appreciated.
