Haven't received a refund for AppleCare+ that was purchased through Verizon but never received / activated.

Over a month ago, I ordered a new iPhone through Verizon and opted to include AppleCare+ (purchased at the same time through Verizon). I was charged for AppleCare+ and the upgrade fee from Verizon for the phone and the taxes. I had to cancel the order 2 days later since the phone was never ready for pickup. Verizon refunded me for the cost of the phone upgrade fee but not for the $199.97 AppleCare+ fee. Since I never picked up the cancelled phone I dont have an AppleCare+ agreement number for AppleCare+ to share with Apple and I am confused how I should go about getting a refund for this charge.
Does anyone have any advice?
