How do I formally dispute charges/file a complaint?

Please send me an email in writing about how to formally dispute charges and file a complaint. 

The call with your representatives at 10:15 am, it was completely inappropriate, un-empathetic, and they told me things completely different than other representatives have said.  

I wont speak with anyone at fios via the phone anymore.
