How to report if a wrong charges are on bill

While I was on a call with customer care, he offered that he is level 4 loyalty expert and I have a free phone eligible on my account and I just have to pay taxes. Convinced me to avail that offer and sent phone. He said he will also have follow-up call for activation. However he never called on said date, and my bill has extra charges and installments plan added. When reached out to customer care they say there is no such offer they are able to see. It was on recorded line and I asked and confirmed multiple times he said itโ€™s free and wonโ€™t charge anything. How do I report and resolve this.
