I am being charged a woefully incorrect amount on my bill, how do I address this?

I have been paying, on time, a bill of $70 every month for some months now. This month, I am being charged $140 on account of an overdue charge of $70; a charge which cannot exist given that I have made all of my payments and on time.
I have checked my payment histor and my bank statements, both of these agree with what I am saying.
I need to address this because I cannot afford to simply pay a double bill that came from nowhere.
