Never Received $200 Visa Gift Card + Netflix Subscription

I signed up for Fios after I received a promo card for $200  Visa and 1 year Netflix subscription. When I called customer service today they claim that the agent never added it to my order. I am very disappointed and dissatisfied by Fios and their lack of integrity when honoring this promo.

I will  be making a corporate complaint and taking this to various news outlet because Verizon has lied.
