Never received $400 gift card

My office moved over the summer, and Verizon promised to send a $400 gift card within a few weeks. It's been six months. Every time I call Verizon to follow up, representatives create tickets which are later closed without a notifying me or giving me a reason. I just want the gift card re-sent and its ETA, both of which Verizon has failed to do at this point.
