I moved states and hence cancelled my Fios service about a month ago on July 14. I only recently received an email acknowledging that I need to return the equipment, but was not provided a label. When I sign into my verizon account online, I don't see an option to print a label. I am also being charged for the internet and TV service for this past month, even though I don't live at the address.
