Redeem Amazon Echo and Prime Subscriptions

I never received an email from Verizon to redeem Amazon Prime Membership or an Amazon Echo device.  I have not recieved any links in the My Verizon or My Fios mobile apps for redemption either.  My account was activated on Jan 2, 1019 and the 60 day redemption window is closing quickly.  How can I redeem this offer?  Thanks!
