Rewards Gift Card

I had opted to use my reward points in order to recieve an Amazon gift card. I tried to apply the code  multiple times into Amazon and I continue to recieve the error message "We had a problem redeeming your gift card. This gift card claim code has been redeemed to another account." 

I don't know why I'm recieving this message since I've never used the card (nor has anyone else) and it definitely hasn't been applied to my account. Can someone help me figure out whats going on?
