Unable to log in for equipment return label

I canceled my Fios service. I was given my Order Number. I followed the link in the email to a Return Equipment page. I entered my Order Number and Zip Code. The Sign In button doesn't do anything. 

How do I get a copy of my return label so I can promptly return the equipment?
