Verizon Owes me money and won't pay

I have been receiving mail from Verizon stating that I am owed money for my cancelled FiOS service. I cancelled this service 2 years ago.

It's not much, but it's still my money. I have contact Verizon before and was assured that the check would be sent. This was over 6 months ago, and nothing yet.

What needs to happen for the refund to actually get to me?
