BYOD $500 rebate/discount not received

We moved 2 lines into our friend's account and were supposed to to receive the $500 rebate , but never received it . Neither we were informed by Store Manager or Customer Support about any activation or follow-up of the rebates, no text message or email sent about the rebate.
Following-up or want to file a complain in the rebate center for false claims and promotions that are not being honored.
